Let's start with a simplified version of the problem -- a foot or hand with only two digits. I'm focusing on just attaching the digits, so I made only a basic extruded spline to represent the appendage, and a very simple digit which I have duplicated once with no scaling. 32 patches all told.
You may be thinking that this looks a lot like a simplified model of a torso and two legs. You're right, and the same technique will work as well for that problem, too!
